Dull abdominal pain thought to be gastritis, turned out to be severe gastrointestinal bleeding
Patient NTA (30 years old) had symptoms of dull pain in the epigastric region, accompanied by black stools, nausea and poor appetite for a week. Thinking it was just a common gastritis, he bought medicine to treat himself at home. However, after 7 days of taking the medicine, the symptoms did not improve, the patient vomited fresh blood and had to be rushed to Hong Ngoc General Hospital.
At the time of emergency, the patient's condition was critical: blood pressure dropped to 80/50 mmHg, pulse rate 100 beats/minute, pale skin, pale mucous membranes... Blood test results decreased severely: red blood cell count, hemoglobin and red blood cell mass decreased to only 1/2 of normal.
Immediately, the patient was resuscitated by the doctor, actively rehydrated, prescribed oxygen, and an emergency transfusion of 700ml of blood to stabilize the patient's condition.
“The patient lost a lot of blood, his vital signs decreased, and the risk of life-threatening hemorrhagic shock could occur at any time. Therefore, we need to stabilize the patient's health first, then perform a digestive endoscopy to find the cause of the bleeding,” said Dr. Nguyen Kim Cuong, Department of Gastroenterology, Hong Ngoc General Hospital.
Through gastrointestinal endoscopy, the medical team discovered an ulcer in the anterior duodenum area with a size of about 2cm that was bleeding. Immediately, the medical team injected the ulcer to stop bleeding and clamped 4 hemostatic clips at the ulcer during the endoscopy. After the intervention, the lesion stopped bleeding, the patient was closely monitored for the first 24 hours and continued with active medical treatment.

After 4 days of intervention, hemoglobin indexes returned to normal, the patient recovered well, had rosy skin, comfortable spirit, could eat and walk normally.
“My health is good now, no more stomachache or fatigue,” patient A shared.
Dull abdominal pain: When should you see a doctor?
