
Pregnant woman BTH (36 years old, from Phu Tho ) was pregnant through in vitro fertilization (IVF), 31 weeks pregnant, hospitalized in a state of severe preeclampsia, central placenta previa and a special weight of 115 kg. This is a particularly high-risk pregnancy, with many potential dangerous complications for both mother and fetus.
With a heavy body weight, cesarean section poses a series of challenges for the treatment team. In pregnant women with special physical conditions, the subcutaneous fat layer is thick, the spine is difficult to determine the anatomical landmarks, making needle puncture for epidural anesthesia difficult and potentially at high risk of failure.
In addition, the thick abdominal wall and limited visibility make the surgery more complicated, leading to increased blood loss and longer surgery times. After surgery, patients are also at higher risk of wound infection, thrombosis and respiratory failure than normal.
On the background of severe preeclampsia and central placenta previa, two diseases that can directly threaten the mother's life, early cesarean section was indicated after interdisciplinary consultation.
The surgical team was directly led by Dr. Do Tuan Dat, Head of the Obstetrics Department A4, with close coordination from the Anesthesia and Resuscitation Department and the Neonatology Department. All anesthesia, resuscitation and neonatal care plans were carefully prepared to ensure maximum safety for mother and child.
After a stressful surgery, the baby boy weighing 2,200 grams was safely delivered. Immediately after crying at birth, the baby was transferred to the Neonatal Department for special care and monitoring due to being born prematurely at 31 weeks.
The mother was monitored and actively resuscitated after surgery. Her health progressed well, without complications. Currently, both mother and child are healthy and have been discharged from the hospital.
According to doctors, being overweight is a factor that increases the risk of obstetric complications, especially when accompanied by diseases such as preeclampsia, placenta previa or gestational diabetes. Therefore, controlling weight, blood pressure and blood sugar before and during pregnancy plays a very important role in preventing complications.
