10th grade student with critical stomach perforation, needs emergency surgery in special zone

(Dan Tri) - Faced with the critical condition of a 10th grade male student's perforated stomach, the patient was urgently consulted with specialists from the mainland, to perform surgery right in the special zone.

On October 5, the leader of the Ho Chi Minh City Department of Health informed Dan Tri reporter that the second rotating group of doctors in Con Dao special zone had just successfully performed emergency surgery on a student with a perforated stomach.

The patient is D.PT (15 years old), a 10th grade student, admitted to the emergency room at Con Dao Military Medical Center with pain in the epigastric region radiating to the right iliac fossa, and worsening after eating.

The male student had a history of gastric ulcer, diagnosed 3 years ago but did not return for a follow-up visit as scheduled by the doctor. After examining the patient's X-ray results, the doctors tended to diagnose a perforated hollow organ.

CT scan showed free air and little fluid in the patient's abdomen with perforation of the gastric pyloric antrum and mesenteric fat infiltration.

Immediately after that, the doctors connected to an online consultation and asked for opinions from the leaders of Binh Dan Hospital. The experts unanimously diagnosed the patient with peritonitis due to a perforated hollow organ, indicated emergency surgery, and agreed to let the doctors perform the surgery right in Con Dao.

The surgical team included Dr. Vu Khuong An, Deputy Head of the Line Command and Scientific Research Department of Binh Dan Hospital, as the main surgeon, while Dr. Nguyen Thi Thanh Truc, Deputy Head of the Department of Anesthesia and Resuscitation 1, was in charge of anesthesia.

In addition, the surgery also had the participation of doctors from Hung Vuong Hospital and Ho Chi Minh City Orthopedic Trauma Hospital.

Through endoscopy, the surgical team discovered a 5mm diameter prepyloric perforation, with a surrounding pseudomembrane and inflammatory fluid in the right abdomen and hypogastric region. The doctors performed a debridement of the perforation, sent a biopsy, sutured the wound, cleaned the abdominal cavity and placed a drain for the patient.

The surgery went smoothly and lasted 60 minutes. After surgery, his health was stable, he was awake, continued antibiotic treatment and was expected to be discharged in the next few days.

"Early detection and treatment when the perforation is still localized has helped patients avoid serious complications such as diffuse peritonitis, sepsis or septic shock," said a member of the surgical team.

According to the Board of Directors of Binh Dan Hospital, after more than 1 month of rotating doctors to Con Dao special zone, the examination, treatment and surgical procedures at the local medical center have stabilized and are increasingly gaining the trust of the people.

Previously, on the first working day of the second rotation of medical staff, the doctors examined a total of 133 outpatients and 3 inpatients, and provided emergency care to 8 cases. Of these, 2 cases required emergency surgery.

Previously, at the end of September, a man named PMQ (67 years old) was transferred to the Con Dao Military Medical Center for emergency treatment in a state of severe abdominal pain, spreading throughout the abdomen after two days of black stools.

The patient was diagnosed with generalized peritonitis, suspected to be due to a perforated gastric prepyloric ulcer, and severe gastrointestinal bleeding. Notably, the man had underlying diseases such as hypertension, ischemic heart disease, pneumonia, and acute kidney injury.

Faced with a situation requiring emergency surgery, Dr. Ly Bao Duy (on his first rotation in Con Dao) connected to a consultation and received remote advice from Associate Professor Tran Vinh Hung, Director of Binh Dan Hospital.

After the consultation, Associate Professor Tran Vinh Hung agreed with the diagnosis and agreed to perform emergency surgery on the patient right in Con Dao.

After surgery, the patient was alert, had no fever, was out of danger, and continued to receive treatment and care according to the correct protocol.
