Ministry of Health proposes to triple allowances for surgeons

The Ministry of Justice has just announced the appraisal dossier of the Draft Decree stipulating a number of special allowance regimes for civil servants, public employees, workers and armed forces in public health facilities, anti-epidemic allowance regime; support regime for medical staff in villages, residential groups and midwives in villages and hamlets. The draft was developed by the Ministry of Health.

According to the assessment of the Ministry of Health , the current regular allowance, epidemic prevention allowance and surgical procedure allowance regime were issued since 2011 with the allowance level being quite high compared to the basic salary at that time.

This allowance has been implemented for 13 years and has not been revised or adjusted, while the basic salary has increased 8 times, with the value doubling from 830,000 VND to 2,340,000 VND.

Deputy Minister of Health Le Duc Luan assessed that the current regulations on the scope of adjustment, beneficiaries and allowance levels have revealed many shortcomings. In particular, the allowance level in specific amounts has been devalued after 13 years without adjustment.

The draft Decree stipulates a number of specific allowance regimes including: On-duty allowance; surgery and procedure allowance; epidemic prevention allowance; monthly support regime for medical staff in villages, residential groups and midwives in villages and hamlets.

Regarding the principle of implementing the on-call regime, the Ministry of Health proposes to add to the group of subjects belonging to special departments and areas: Stroke, organ transplant, delivery room, intensive care at special-class, class I, class II hospitals; public emergency facilities outside public hospitals; care areas for patients with compulsory treatment for mental illness in hospitals and institutes.

Supplementing the on-call regime at public forensic forensic examination organizations; the on-call regime at the National Coordination Center for Human Organ Transplantation; the on-call regime at medical facilities established under the provisions of law on handling administrative violations; facilities for caring for war invalids and sick soldiers; and public social assistance facilities.

According to the proposal of the Ministry of Health, employees (doctors, nurses...) on duty 24/24 hours on working days are entitled to an allowance of 325,000 VND/person/shift for: Special class medical facilities (such as Bach Mai Hospital, Viet Duc Hospital, Cho Ray Hospital, Thai Nguyen Central Hospital...), class I (most central hospitals and provincial hospitals); Central Institute of Forensic Psychiatry, Bien Hoa Central Institute of Forensic Psychiatry.

The level of 255,000 VND/person/shift is applied to level II medical facilities, the National Institute of Forensic Medicine, regional forensic psychiatric centers, the Military Institute of Forensic Medicine under the Ministry of National Defense, the Criminal Technical Examination Department under the Ministry of National Defense, the Forensic Examination Center under the Institute of Criminal Science, the Ministry of Public Security, the Criminal Science Institute under the Ministry of Public Security, the Criminal Technical Department under the Provincial Police, the National Coordination Center for Human Organ Transplantation (for on-duty coordination of taking, transporting, and preserving human tissues and organs).

The rate is 185,000 VND/person/on-duty for the remaining medical facilities, provincial forensic centers, the National Coordination Center for Human Organ Transplantation (for on-duty consultation and mobilization of human tissue and organ donation), and out-of-hospital emergency facilities (for on-duty at emergency points).

Level 70,000 VND/person/shift for: commune health stations, health stations under commune health stations, military and civilian health stations, military and civilian infirmaries, infirmaries of the people's police, medical facilities established according to the provisions of law on handling of administrative violations, facilities for caring for war invalids and sick soldiers, public social assistance facilities.

If on duty at a department or special area as prescribed, the on-duty allowance is calculated at 1.5 times the above prescribed rate. If on duty on a weekly day off, the on-duty allowance is calculated at 1.3 times the above prescribed rate; if on duty on holidays or Tet, the on-duty allowance is calculated at 1.8 times the above prescribed rate.

Regarding the surgical and procedural allowance regime, the Ministry of Health proposes that all specific allowance levels for each group of subjects and each type of surgery be adjusted to increase by 182% compared to the current prescribed allowance level.

Specifically: For special surgeries, the allowance for the main surgeon, anesthesiologist or main anesthetist is proposed to increase nearly 3 times, from 280,000 VND to 790,000 VND. The new allowances for type I, II, and III surgeries are 355,000 VND, 185,000 VND, and 140,000 VND, respectively.

Surgical assistants and anesthesiologists in special surgeries can receive 565,000 VND. Special surgery assistants are recommended to receive 340,000 VND. For medical procedures, the allowance will be 30% of the same type of surgical allowance.
