62-year-old man in Tuyen Quang had acute liver failure after drinking alcohol soaked with medicinal leaves

On December 22, information from Thai Nguyen Central Hospital said that recently, doctors at the Hospital's Emergency Department received and treated a patient with acute liver failure due to toxic hepatitis.

Accordingly, patient TVT (62 years old, in Yen Son, Tuyen Quang ) has a history of 2 coronary artery stents and taking prescription medications as an outpatient, had gastric perforation surgery 40 years ago, and has no history of liver disease.

According to prehistoric information, Mr. T was introduced to the effects of some types of leaves by the villagers, so he went to get them himself, cooked them into a paste, and soaked them in alcohol.

One month before being hospitalized, Mr. T used about 500ml of wine made from the leaves every day. After a period of use, Mr. T felt increasingly tired, with yellow eyes and skin.

After a week of treatment at a local private hospital, seeing no improvement in his condition, his family immediately transferred Mr. T to Thai Nguyen Central Hospital.

Here, the patient's consciousness was impaired, lethargic; the skin, mucous membranes, and sclera were dark yellow. Blood test results showed very high liver enzymes (SGOT/SGPT - 4438/4705UI/l; GGT - 271.1U/l; Total Bilirubin/TT - 268.4/152.7µmol/l; Protein/Albumin: 70/37 g/l; Ammonia 79.9 µmol/l); HbsAg and HCV Ab were both negative.

He was diagnosed with acute liver failure and was placed on observation due to toxic hepatitis. Doctors performed plasma exchange, intensive resuscitation to support the liver, prevent cerebral edema, and combine measures to reduce blood ammonia and ensure respiration and nutrition.

After 6 days of intensive treatment, the patient's health improved significantly, he ate well, slept well, his skin was no longer yellow, and his test results returned to near normal limits. The patient was discharged from the hospital.

Doctors would perform basic resuscitation measures for patients such as circulatory resuscitation; treatment against cerebral edema, blood clotting disorders and complications while waiting for liver cells to recover or waiting for liver transplant surgery; prevention of gastrointestinal bleeding...

With the development of science and technology and the expertise of the medical staff, since 2009, plasma exchange therapy has been performed at Thai Nguyen Central Hospital. Up to now, this therapy has been an active therapy in the treatment of acute liver failure, helping to eliminate toxins produced during the metabolism process, supporting the liver while waiting for liver function to recover.
