12 years old but weighing 83kg, the 12 year old boy suffered from blood clotting disorder, gastrointestinal bleeding, liver damage, and severe respiratory failure when he contracted dengue fever.

On July 7, Dr. Nguyen Minh Tien - Deputy Director of the City Children's Hospital (HCMC), said that he had just received treatment for a boy TNMK (12 years old, living in HCMC) with dengue shock syndrome due to being overweight and obese, weighing 83kg (at this age, the weight is about 34 - 36kg).

Medical history showed that K. had a high fever for 3 consecutive days. On the 4th day, he had a stomachache, vomited brown curd, and had cold hands and feet, so his family took him to the City Children's Hospital.

Here, baby K. was diagnosed with severe dengue shock on day 4, blood clotting disorder, gastrointestinal bleeding, liver damage, and severe respiratory failure in an overweight and obese child.

Doctors gave the child an infusion of high molecular weight dextran 40 10%, albumin 10%, anti-shock, used vasopressors in combination, respiratory support with continuous positive airway pressure, and non-invasive ventilation.

However, the patient still had blood clotting disorder and gastrointestinal bleeding, so he received blood transfusion, fresh frozen plasma, cryoprecipitate, platelet concentrate, vitamin K1, and liver support treatment.

After nearly 7 days of treatment, the patient gradually recovered, became alert, breathed fresh air on his own, and his liver and kidney function returned to normal.

Referring to world medical literature and through treating many cases of dengue shock, doctors found that cases often progress severely, abnormally, causing difficulties in treatment such as: children with overweight and obesity, infants, early symptoms of dengue shock (day 3, 4 of the disease), high blood concentration...

Through the above case, Dr. Tien advised parents to take their children to the doctor early when they have a high fever for more than 2 days so that doctors can examine them, accurately diagnose the disease and have a specific treatment plan.

When a child has one of the following signs, he or she must be taken to the hospital immediately, even at night: restlessness, tossing and turning or lethargy, drowsiness, delirium, nosebleeds, bleeding gums or vomiting blood, black stools, abdominal pain, vomiting, cold hands and feet, lethargy, lying in one place, not playing or refusing to breastfeed or eat or drink.

Dr. Tien also noted that parents need to pay attention to a reasonable diet according to their age, and it is best to consult a nutritionist to avoid the risk of being overweight or obese because treating dengue fever in these children is very difficult, and they are susceptible to complications such as respiratory failure, blood clotting disorders, liver and kidney damage, etc.

Take proactive measures to prevent dengue fever

The Ho Chi Minh City Department of Health calls on every citizen to proactively take measures to prevent dengue fever, including: covering water containers, cleaning, removing water sources and waste around the house; sleeping under mosquito nets and using mosquito repellents.

When experiencing fever symptoms, people should go to a medical facility for examination and not self-treat at home. At the same time, actively cooperate with the health sector in mosquito larvae eradication and chemical spraying campaigns.

There is now a vaccine to prevent dengue fever for children 4 years of age and older and adults.
