E Hospital ( Hanoi ) said that recently, doctors admitted a 34-year-old male patient (in Thanh Xuan District, Hanoi) to the emergency room with severe pain in the lumbar region. What is special is that after examination, doctors discovered that the patient had 4 kidneys in his body.

Dr. Nguyen Dinh Lien, Head of the Department of Urology and Andrology, Hospital E, said the patient was admitted to the hospital with severe pain in the lumbar region, abdominal distension, painful urination, and blood in the urine.

The patient was ordered by the doctors to do tests, abdominal ultrasound, and CT scan. The doctors determined that the patient had a stone in the lower third of the right ureter (located at the junction of the ureter when it enters the bladder), the stone was approximately 9 x 7 mm in size.

More surprisingly, through CT scan, doctors discovered that the patient had completely double kidneys and ureters on both the left and right sides, and two separate ureters both inserted into the bladder.

"This is a case of abnormal urinary system anatomy, with 4 kidneys instead of the normal 2, which is one of the causes of kidney stones," a doctor assessed.

Exploiting the patient's medical history, the doctors learned that due to the nature of his job, the patient often had to drink alcohol, but each time he drank alcohol, the patient had cramps and even had blood in his urine, so he bought medicine to treat himself at home. Recently, the pain symptoms caused by kidney stones increased and became more severe, so the patient went to E Hospital for examination and treatment.

Regarding "possessing" 4 kidneys in the body, the patient said that he only discovered this 7 years ago, when his daughter was born and doctors discovered an abnormality in her body with 3 kidneys and 2 bladders (one large bladder and one small bladder). Doctors removed a small bladder from his daughter when she was 18 months old and reconstructed her urinary system. At the same time, doctors advised everyone in the family to screen the urinary system and discovered that he had 4 kidneys in his body.

Suddenly discovered living 51 years with only 1 kidney

According to Dr. Lien, this is a rare congenital defect caused by the abnormal development of the left ureter during fetal formation and development.

The patient has 4 ureteral openings, 2 right ureteral openings and 2 left ureteral openings. Of which, the right ureteral opening of the lower renal unit is near the bladder neck, the left ureteral opening of the left renal unit is swollen and inflamed, making it difficult for doctors to find the ureteral opening to directly access the stone.

In the case of the male patient mentioned above, Dr. Lien shared that the difficulty in treatment was because the patient had more kidneys than normal people. The patient had kidney stones and self-treated for a long time but was ineffective, so inflammation and edema formed at the ureteral opening.

Doctors chose the intervention option using retrograde endoscopic laser lithotripsy of ureteral and kidney stones.

This is a technique that uses an endoscope from the urethra, through the bladder and up the ureter to directly access the stone. Then use compressed air or laser energy to break the stone and pump and pick it up to remove all the stone fragments. This method has advantages such as: less invasive, protecting kidney function. During the treatment, the patient feels less pain and recovers quickly.

After surgery, the male patient's kidney stone samples were dissected, thereby providing a further treatment plan to limit the risk of stone formation again.
