Effectiveness from inter-hospital coordination

Remote consultations and surgeries supported by provincial doctors have become effective solutions, contributing to improving professional capacity, shortening emergency time and increasing the patient's chance of survival.

The Van Don Special Zone Medical Center has just coordinated with the Provincial General Hospital to successfully perform surgery to remove a chronic subdural hematoma in the left hemisphere for patient NVM (76 years old, residing in Van Don). Previously, the patient was admitted to the hospital with weak limbs, unsteady walking, and abnormal symptoms that lasted for many days. Through clinical examination and brain CT scan, doctors discovered a subdural hematoma in the left hemisphere, a common type of injury in the elderly, with many potential risks of serious complications. Recognizing this as an emergency surgical situation, the Center immediately invited provincial doctors to assist with the surgery.

Doctor CKII Nguyen Tien Dung, Deputy Director of the Provincial General Hospital, directly performed the surgery with the medical team of the Center. After nearly 2 hours of surgery, the hematoma was completely removed, the patient was awake, and neurological symptoms improved significantly.

Doctor CKII Hoang Huy Toan, Head of the Department of Surgery - Maxillofacial - Eyes - Ear, Nose and Throat (Van Don Special Zone Medical Center), said: Receiving timely support from the provincial level not only helps save patients' lives, but is also an opportunity for the team at the grassroots level to improve their professional skills and gradually master advanced techniques.

A cardiovascular intervention at the Provincial General Hospital.

On the evening of July 6, 2025, Cam Pha Regional General Hospital received a 73-year-old patient (Cua Ong ward) in a state of dizziness, fatigue, dull pain in the left chest, and difficulty breathing when exerting. Although the initial indicators were relatively stable, the doctor on duty promptly ordered an electrocardiogram, detecting acute inferior myocardial infarction with third-degree atrioventricular block, a dangerous disease that can easily lead to death if not intervened promptly. The team immediately deployed emergency care on the spot, concurrently held an online consultation with the Provincial General Hospital and activated an emergency transfer. Thanks to the smooth coordination process, the patient was safely transferred to the hospital and timely coronary intervention was performed. The successful stent placement helped save the patient's life at the last minute.

Not only is the emergency effective, the inter-hospital coordination model also helps handle many complex, high-tech cases right at the grassroots level. A typical example is the surgery to treat grade III genital prolapse for a 77-year-old patient, which was successfully performed at Quang Yen Medical Center. The patient was admitted to the hospital in a serious condition, with the uterus almost completely prolapsed out of the vagina, accompanied by many complex underlying diseases such as cardiovascular disease, high blood pressure, etc., increasing the risk of complications if not handled properly. In this situation, the Center proactively activated the inter-hospital consultation mechanism, inviting experienced obstetricians from Bai Chay Hospital to participate in the coordination.

The surgical team, consisting of obstetricians, surgeons, and anesthesiologists from the two units, chose the Crossen method, a modern technique that requires high precision, combined with modern support equipment such as C-Arm irradiation. After nearly 3 hours, the surgery was performed safely, without complications, helping the patient recover well after surgery.

The inter-hospital coordination model in Quang Ninh is creating a clear change in improving the professional capacity of primary health care. Instead of transferring all patients, many patients are treated and given effective emergency care on the spot, thanks to professional support programs, continuous training, and technology transfer from upper-level hospitals to lower-level hospitals.

Currently, the Medical Centers of Van Don, Hai Ha, Quang Yen, Mong Cai... have mastered many techniques, such as brain surgery, emergency abdominal surgery, intensive care, cardiovascular emergency... Regular support from provincial hospitals helps the grassroots medical team confidently handle more complicated cases, improving the quality of treatment for the people.

The provincial health sector continues to expand the inter-hospital coordination model, deploy new techniques at lower levels, apply technology in remote consultation, and share medical data. This not only reduces the burden on the provincial level, but also helps people, especially in remote areas, to access quality, safe and economical medical services right where they live.
