Patient NHV (50 years old, in Hanoi ) had a history of type 2 diabetes, hypertension, lipid metabolism disorder, chronic kidney disease stage IIIA, heart failure, chronic hepatitis C. One month before, the patient had been treated for slow-healing ulcer of the right foot stump at the Central Endocrinology Hospital.

According to the family, the patient started to feel drowsy and gradually lost consciousness. A capillary blood sugar test showed a result of 2.2 mmol/l. Although he was given sugar water at home, the patient did not improve. The patient was then taken to the Central Endocrinology Hospital.

Doctors at the Intensive Care Unit said that upon admission, the patient was in a deep coma (Glasgow coma scale = 8 points), with signs of neck stiffness and meningitis. Clinical examinations and imaging tests showed that the patient had an intracranial hemorrhage on the basis of many chronic diseases.

Upon admission, the team of doctors from the Intensive Care Unit quickly performed endotracheal intubation, anti-cerebral edema, blood pressure control, and closely monitored the patient's condition. After initial resuscitation, the patient was transferred to a specialist.

Here, the hospital's neurosurgeons consulted and decided to perform open skull surgery, ventricular shunt to reduce brain pressure, and then continue treatment at the Surgical Intensive Care Department.

According to endocrinologists, long-term diabetic patients, especially those with multiple comorbidities, are at high risk of serious complications. Therefore, early detection, timely treatment and close coordination between specialized departments play a key role in saving and preserving the patient's life.

The Central Endocrinology Hospital recommends that diabetic patients should have regular check-ups, comply with treatment, monitor blood sugar regularly and not self-treat when there are unusual signs. In case of emergency, they should be taken immediately to a medical facility for timely emergency care.
