Proposal to increase special allowances for civil servants, public employees, and armed forces in public health facilities

According to the latest proposal of the Ministry of Health, at special medical facilities, medical staff on duty 24/7 will receive an allowance of 325,000 VND for each shift; at the same time, the surgical allowance will also increase, with the highest being 790,000 VND.

The Ministry of Justice has just announced the appraisal dossier of the draft Decree stipulating a number of special allowance regimes for civil servants, public employees, workers and armed forces in public health facilities, anti-epidemic allowance regime; support regime for medical staff in villages, residential groups and midwives in villages and hamlets. The draft was developed by the Ministry of Health.

The draft Decree consists of 5 chapters and 10 articles stipulating a number of specific allowance regimes including: on-duty allowance; surgery and procedure allowance; epidemic prevention allowance; monthly support regime for medical staff in villages, residential groups and midwives in villages and hamlets.

The allowance level of health workers has remained unchanged for 13 years, while the basic salary has increased 8 times.

According to the Ministry of Health, although the current regular allowance, epidemic prevention and control allowance and surgical procedure allowance were issued in 2011 with a fairly high allowance level compared to the basic salary at that time.

However, up to now, after 13 years of implementation, there has been no amendment or adjustment while the basic salary has increased 8 times with the value doubling from 830,000 VND to 2,340,000 VND.

Current regulations on the scope of regulation, beneficiaries and allowance levels have revealed many shortcomings. For example, the name of the permanent allowance is no longer consistent with the Law on Medical Examination and Treatment; the names of some organizations receiving allowances have changed in other documents; some beneficiaries have not been updated...

In particular, the allowances in specific amounts have been devalued after 13 years without adjustment. Therefore, these allowances need to be studied, revised and supplemented to suit the reality.

The Ministry of Health believes that the issuance of a Decree regulating a number of special allowance regimes for civil servants, public employees, and workers in public health facilities, anti-epidemic allowance regimes; support regimes for medical staff in villages, residential groups, and midwives in villages and hamlets at present is necessary.

That is the basis for localities and public health facilities to issue regulations and implement a number of special allowance regimes for civil servants, public employees, and workers in public health facilities, anti-epidemic allowance regimes, and support regimes for medical staff in villages and residential groups; and midwives in villages and hamlets to ensure the stable maintenance of human resources to perform the task of taking care of people's health.

Proposal to increase support level for primary health care

According to the Ministry of Health, the notable highlight of this draft Decree is the conversion of "village and hamlet health worker allowance" into "monthly support regime" and the expansion of its application to health workers in residential groups in wards and towns.

Currently, according to Decision 75/2009/QD-TTg, the allowance is only 300,000-500,000 VND/month, depending on the locality, and is paid from the local budget.

The Ministry of Health proposed to increase the monthly support regime for village and hamlet midwives.

The latest draft increases the support level to 0.6 to 1.0 times the basic salary, or about 1.4-2.3 million VND/month when calculated according to the new basic salary of 2.34 million VND applied from July 1, 2024. For village midwives - the force specializing in maternal and child health care in remote areas, the support level can be up to 1.2 times the basic salary, or nearly 2.8 million VND/month.

According to the Ministry of Health, the new support level is expected to increase 1.5-2 times compared to the current level (currently at 400,000-500,000 VND/month), to help them maintain their lives and feel secure in their work.

The draft Decree also adds many new groups of subjects: officers, soldiers, police, defense workers working in medical facilities of the armed forces; forensic and forensic psychiatric organizations; public social assistance facilities caring for the sick, wounded and sick soldiers. These are forces performing public health tasks but have not received special allowances for many years.

All allowances and supports are calculated based on the new basic salary of VND 2.34 million/month, according to Decree 73/2024/ND-CP dated June 30, 2024 of the Government stipulating the basic salary and bonus regime for cadres, civil servants, public employees and armed forces.

Proposed level of direct allowance regime

According to the draft Decree, the head of the medical examination and treatment facility shall base on the actual situation to decide on the form of arrangement of employees to work in shifts, shifts or overtime. In case of shortage of human resources, it is impossible to arrange employees to work in shifts or overtime, then employees must be arranged to be on duty 24/24 hours.

Regarding the on-duty allowance regime, the draft clearly states that employees on duty 24/24 hours a day are entitled to an allowance of 325,000 VND/person/shift. This allowance is applied to special-class and class I medical facilities; the Central Institute of Forensic Psychiatry, the Central Institute of Forensic Psychiatry, Bien Hoa.

The allowance of 255,000 VND/person/shift applies to: class II medical facilities, National Institute of Forensic Medicine, Regional Forensic Psychiatric Center, Military Forensic Medicine Institute under the Ministry of National Defense, Criminal Technical Appraisal Department under the Ministry of National Defense, Forensic Appraisal Center under the Institute of Criminal Science, Ministry of Public Security...

The allowance of 185,000 VND/person/shift applies to: remaining medical facilities, provincial forensic centers, the National Coordination Center for Human Organ Transplantation (for on-call consultation and mobilization for human tissue and organ donation), and out-of-hospital emergency facilities (for on-call at emergency points);

Commune health stations, health stations belonging to commune health stations, military and civilian health stations, military and civilian infirmaries, infirmaries of the people's police... apply an allowance of 70,000 VND/person/shift.

For employees on duty under the permanent outpatient regime, they are entitled to the following allowance: 160,000 VND/person/shift for special-class and class I medical facilities; Central Institute of Forensic Psychiatry, Central Institute of Forensic Psychiatry Bien Hoa.

Grade II medical facilities, National Institute of Forensic Medicine, regional forensic psychiatric centers, Military Institute of Forensic Medicine under the Ministry of National Defense, Criminal Technical Examination Department under the Ministry of National Defense, Forensic Examination Center under the Institute of Criminal Science, Ministry of Public Security... apply the rate of 125,000 VND/person/shift.

The remaining medical facilities, provincial forensic centers, the National Coordination Center for Human Organ Transplantation (for on-call consultation and mobilization of human tissue and organ donation), and out-of-hospital emergency facilities (for on-call at emergency points) will have an allowance of VND 90,000/person/shift.

For commune health stations, health stations belonging to commune health stations, military and civilian health stations, military and civilian infirmaries, infirmaries of the people's police, medical facilities established under the provisions of law on handling administrative violations, facilities for caring for war invalids and sick soldiers, and public social assistance facilities, the rate of VND 35,000/person/shift will be applied.

The highest surgical allowance increased to 790,000 VND.

According to the latest proposal of the Ministry of Health, the highest increase (special type) for the main surgeon, anesthetist or main sedative is 790,000 VND, an increase of 510,000 VND (currently 280,000 VND); type 1 for this subject is 355,000 VND, an increase of 230,000 VND compared to current; type 2 for this subject is 180,000 VND, an increase of 120,000 VND; type 3 for this subject is 140,000 VND, an increase of 90,000 VND compared to current.

For surgical assistants, anesthesiologists or special anesthesia assistants, the salary is 565,000 VND, an increase of 365,000 VND compared to the current one; type 1 is 255,000 VND, an increase of 160,000 VND compared to the current one; type 2 is 140,000 VND, an increase of 90,000 VND compared to the current one; type 3 is 85,000 VND, an increase of 55,000 VND compared to the current one...

For direct surgical assistants, the special type increases from 120,000 VND at present to 340,000 VND; type 1 increases to 195,000 VND compared to 70,000 VND at present; type 2 increases to 85,000 VND compared to 30,000 VND at present; type 3 increases to 40,000 VND compared to 15,000 VND at present.

The draft regulation stipulates that workers on duty 24/24 hours will receive a meal allowance of 40,000 VND. In case of working 24/24 hours on weekdays and weekly holidays, they will get 1 day off in compensation; those on duty 24/24 hours on holidays and Tet will get 2 days off in compensation...

The draft also proposes to increase the anti-epidemic allowance regime for both groups 1 and 2.

According to the Ministry of Health, the new draft Decree will affect about 640,000 people in the entire public health system, including 520,000 officials and workers at hospitals, medical centers, and preventive facilities, and 120,000 medical staff in villages, hamlets, residential groups, and village midwives.
