Previously, at 11:30 a.m. on August 31, a male patient named NVH (39 years old) was brought to the Emergency Department of the Northern Mountainous Region General Hospital of Quang Nam in a state of cardiac arrest, respiratory arrest, cyanosis of the whole body, and no pulse. The on-duty team quickly intubated, performed cardiopulmonary resuscitation, and performed electric shock to defibrillate.
After more than 30 minutes of emergency efforts, the patient's heartbeat returned. Doctors diagnosed the patient with: "Acute myocardial infarction complicated by arrhythmia (ventricular fibrillation), respiratory arrest". Immediately after, the patient was treated with a ventilator, maintained on vasopressors and antiarrhythmic drugs.
This was determined to be a very critical case of acute myocardial infarction: blood pressure dropped to 70/50 mmHg despite taking 3 high-dose vasopressors at the same time, and there was persistent ventricular arrhythmia. The patient was urgently transferred to the Cardiovascular Intervention Unit for emergency coronary angiography. The coronary angiography results showed complete occlusion of the proximal anterior interventricular artery.
The intervention team quickly placed a stent to re-open the blocked coronary artery within 15 minutes. After successful re-opening, the patient continued to be treated at the Intensive Care and Anti-Poison Department. At this time, test results showed that the patient was in a state of severe metabolic acidosis with acute renal failure - a consequence of respiratory and circulatory arrest.
The patient continued to be ventilated, maintained on vasopressors, antiarrhythmic drugs and continuous renal replacement therapy (CRRT). After receiving active and effective treatment, the patient's condition gradually improved.
After more than 20 hours since being admitted to the hospital, the patient was taken off the ventilator, the dose of vasopressors was reduced, he could breathe oxygen on his own, he was more alert and able to communicate. By September 3, the patient no longer had chest pain or difficulty breathing, was completely alert and could walk easily.
