On the afternoon of September 29, the team of doctors at the Central Obstetrics Hospital successfully performed a special cesarean section for a 35-year-old pregnant woman with stage 3 nasopharyngeal cancer. This was a special case, full of stress, challenges, and also filled with the young mother's determination and sacred maternal love.
The pregnant woman is Ms. BTMT (35 years old, Hai Phong ) who is pregnant for the third time. Three months ago, she was diagnosed with nasopharyngeal cancer stage CT2N3M0 (a 4cm tumor in the neck area and has spread to the supraclavicular lymph nodes) when she was 22 weeks pregnant.
When she discovered the disease, Ms. T. was advised to undergo chemotherapy and keep the pregnancy at K Hospital. However, because she was afraid of affecting her child, she agreed not to undergo treatment, accepting all risks so that the fetus could grow. Keeping her worries in her heart, she held back her emotions and confided: "When I discovered the disease, I was very worried, but I still tried for the sake of my child so that he could be born safely."

For 3 months, the disease became more and more serious, the tumor grew rapidly, making it difficult for her to swallow, difficult to speak, had to lie in a high head position, poor nutrition, and slow fetal development. A week before the surgery, she was hospitalized in the Department of Obstetrics and Gynecology, the baby was injected with lung maturation to prepare for the "brain-weighing" cesarean section.
At 35 weeks of pregnancy, the mother's endurance had "reached its limit", an interdisciplinary consultation and hospital leadership decided to perform a cesarean section to ensure the safety of both mother and child.
The surgery was performed by Dr. Nguyen Thai Giang - Deputy Head of the Department of Gynecological Oncology.
“This is the first case we have encountered where a pregnant woman had advanced nasopharyngeal cancer. The patient sacrificed the opportunity for early treatment to keep her child. When she returned, her condition was very serious, almost exhausted. At 35 weeks of pregnancy, the mother's health had reached its limit, we were forced to hold an interdisciplinary consultation and decided to perform a cesarean section,” Dr. Giang confided.

The biggest difficulty in the operating room was anesthesia and resuscitation. The large neck tumor prevented the mother from lying on her back, making it difficult to place an IV line and intubate, with increased blood vessels and a high risk of bleeding. The team had to operate in the Fowler position and prepare for any eventuality.
Master, Specialist II Doctor Bach Minh Thu - Head of the Department of Anesthesia and Resuscitation Surgery shared that the team had to choose the spinal anesthesia option and prepare all backup scenarios.
"The patient could only half lie and half sit. It was an extremely challenging situation, requiring close coordination from the entire team," Dr. Thu shared.
After a stressful time, with the smooth coordination of the entire surgical team, the surgery was successful. The baby boy was born safely, weighing 2,300 grams, rosy, healthy, crying loudly in the emotional outburst of the mother and the medical team.
Witnessing the mother's determination to keep the pregnancy despite her illness, Dr. Nguyen Thai Giang was moved to share: "I truly admire this mother's determination, endurance, patience and boundless love. For 3 months, the mother persevered, endured pain, just hoping for her baby to be born healthy. It was a noble sacrifice that no words can describe."

Dr. Giang affirmed that Ms. T's case is a testament to the effective coordination between many specialties, and emphasized the importance of early detection of cancer during pregnancy.
