Rare: Double surgery to deliver a 37-week fetus and remove an inflamed appendix

On July 5, information from Danang Hospital said that doctors had successfully performed a double cesarean section and removed an inflamed appendix, ensuring the safety of both mother and baby.

Previously, on June 28, pregnant woman NLT (30 years old, residing in Da Nang City) had a dull pain in the epigastric region, so her family took her to the Emergency Department of Hoan My Da Nang Hospital for examination.

At the time of admission, the pregnant woman T. was 37 weeks pregnant. MRI results showed that the pregnant woman had acute appendicitis, fluid-filled intestine, edema, and surrounding infiltration.

During the process of monitoring the appendicitis, the fetal heart chart simultaneously recorded signs of premature labor. Uterine contractions 2 times/10 minutes, strong intensity.

Faced with the risk of an inflamed appendix bursting at any time, endangering the lives of both the pregnant woman and the fetus, after an interdisciplinary consultation, the doctors decided to perform a cesarean section first, then remove the appendix.

"The purulent appendix was in a complicated position in a pregnant woman who was showing signs of premature labor, 37 weeks pregnant with a weight that reached the threshold. Considering the benefits for the mother and fetus, the team decided to perform a cesarean section and treat the appendix in one surgery," shared Master - Doctor Ho Thuyen, Department of Obstetrics and Gynecology.

Two surgical teams were deployed simultaneously. After 45 minutes, the surgery was completed safely. The fetus was born weighing 2.7 kg and had a rosy complexion. The purulent appendix was promptly removed, with no post-operative infection complications. Both mother and child were healthy and were discharged after 3 days of observation.

According to Dr. Ho Thuyen, appendicitis in pregnant women is quite rare, with a rate of 1/2,000. Depending on the time, the doctor will consider the optimal treatment method for the mother and fetus.

Due to anatomical changes during pregnancy, signs of appendicitis are often faint and are likely to be obscured by common signs of the postpartum period. Patients are easily subjective, leading to many dangerous complications such as peritonitis, appendiceal abscess, etc.

Pregnant women should not self-monitor at home if they have abdominal pain, especially localized, dull pain in the right abdomen or epigastric region, which does not subside after resting. Early and complete examination helps to promptly detect potential dangerous causes.
