Difficulty urinating, frequent urination, examination revealed penile urethral stricture

Patient NQK (29 years old, in Ho Chi Minh City) was hospitalized with difficulty urinating, pain and swelling in the left testicle, dull and increasing over 2 weeks with mild fever.

Taking medical history, the patient said that he had previously had symptoms of painful urination, difficulty urinating, swollen left scrotum, and pain when touched. The patient went to a medical facility for examination but his condition became more serious and he was admitted to Nguyen Trai Hospital for emergency treatment.

At the emergency department, after examination and consultation at the department, the doctor concluded that patient K. had left testicular and epididymitis and transferred him to the Urology department for treatment.

Specialist Doctor Phan Lien Khuong, Head of the Department of Urology, Nguyen Trai Hospital, said that the patient was treated with intravenous antibiotics combined with strong anti-inflammatory and painkillers for 7 days, the left testicle was less swollen, no more pain, no fever. However, the patient still complained of difficulty urinating, had to strain to urinate and felt like he could not urinate completely.

The patient was prescribed retrograde cystourethrography and urinary cystourethrography. Through examination and medical history, the doctor diagnosed penile urethral stricture.

Oral mucosal grafting for urethral stricture treatment

The team decided to perform anterior urethroplasty using buccal mucosa grafting. The surgeon made an incision around the glans, dissecting the narrowed urethra. A 1.5x6 cm cheek mucosa graft was taken, processed, and then sutured to the base of the corpus cavernosum to recreate the new urethra. After that, the patient had bleeding stopped, the corpus spongiosum and foreskin were sutured closed, and the grafted area was bandaged.

According to Dr. Khuong, the oral mucosal grafting technique in the treatment of anterior urethral stricture is a complex technique with many stages and requires meticulousness and delicacy. In cases of long penile urethral stricture, which cannot be surgically resected, the autologous grafting method has many advantages such as good results, high success rate according to medical literature (82-89%).

Master - Doctor Vo Duy Tam, Nguyen Trai Hospital said that urethral stricture can be caused by many different reasons, from congenital, autoimmune to acquired (such as after trauma, after urethritis, due to medical intervention, ...) but all lead to a common consequence of difficulty urinating and other urinary disorders.

If you currently feel urinary disorders, especially urinary tract obstruction symptoms such as difficulty urinating, straining to urinate, prolonged urination, weak or small urine stream, incomplete urination, etc., you should go to the doctor to be examined and screened for urethral stricture as well as many causes of urinary tract obstruction. Especially if there are risk factors such as a history of urethritis, a history of genital trauma, or previous medical interventions (endoscopic urinary tract surgery, urethral catheter placement, etc.).

"Do not be subjective with symptoms of difficulty urinating because they can lead to complete loss of bladder function as well as affect the urinary tract, potentially leading to kidney failure," Dr. Tam warned.
