Multiple organ failure due to common disease during rainy season

The Central Hospital for Tropical Diseases has just admitted a female patient PTK (46 years old, residing in Hai Phong) with a continuous high fever of 39-40oC, blood pressure dropping to 80/50 mmHg, quickly turning into shock, multiple organ failure. The patient was diagnosed with a suspected infection caused by Leptospira - a bacteria commonly found during the rainy season.

Ms. K. is a worker at a private leather shoe factory. She often coughs and has difficulty breathing when the weather changes. She has a history of high blood pressure for 4 years, takes medication daily, has no liver or gallbladder disease, and does not use traditional Chinese medicine.

About 4 days before being admitted to the hospital, she appeared very tired, her eyes and skin were yellowing, and she had diarrhea many times, so her family took her to a nearby hospital for examination and treatment. There, she was diagnosed with gallstones, treated for 1 day, and then asked to be discharged. The next day, she had a high fever of 40 degrees Celsius, her body was exhausted, so her family transferred her to the Central Hospital for Tropical Diseases.

She was admitted to the Hepatitis Department with a diagnosis of acute hepatitis with sepsis, blood pressure dropped to 80/50mmHg, and was immediately transferred to the Intensive Care Center.

Master, Doctor Tran Van Quy (Intensive Care Center) said that when admitted, the patient was conscious and able to breathe on his own, but had warm crackles in both bases of the lungs. The patient was given oxygen support, but his blood pressure was still low (90/60mmHg), his fever was 39 degrees Celsius, he had no edema, and he did not need to use vasopressors.

Immediately after that, the patient developed metabolic disorders, quickly fell into shock, acute multiple organ failure. Test results showed elevated liver enzymes 1,068 U/L, bilirubin 605 µmol/L, infection index >100. The patient was treated intensively, sedated, intubated, continuous blood filtration, and given antibiotics due to suspected biliary tract infection and Leptospira infection (yellow spirochetes) with a poor prognosis.

Dr. Quy emphasized that Leptospira is an infectious disease caused by spiral-shaped bacteria in humans and animals. In humans, the disease can cause a series of symptoms, and if not diagnosed and treated promptly, it will cause life-threatening complications.

Leptospira infections often progress rapidly, with an average treatment time of 10 to 14 days for patients who respond well to medication.

Doctor Quy warned that in the context of the rainy season causing flooding in many areas, the risk of Leptospira bacteria existing and spreading more strongly. To prevent the disease, people should limit direct contact with flood water, stagnant ponds, canals; if forced to go into the water, they should wear protective clothing, rubber boots and gloves. Scratches on the skin must be carefully covered and cleaned immediately after leaving the flooded area.

Water sources for daily life must be safe, do not use contaminated water for drinking, must be boiled or disinfected with Cloramin B before use. After cleaning, contact with flooded environment, wash hands with soap, shower and pay attention to control rats - the main source of pathogens - as well as keep the environment around the house clean.

When suspicious symptoms such as high fever, muscle aches, yellow eyes, yellow skin appear, people should immediately go to a medical facility for timely diagnosis and treatment.
