Two years ago, the patient was diagnosed with bladder cancer, underwent a total cystectomy, and had two percutaneous ureteral catheters inserted. After surgery, the patient had a JJ catheter inserted from the renal pelvis to the ureter to drain urine while waiting for the ureter to recover from the damage. The JJ catheter needed to be removed within three months, but the patient did not return for a follow-up examination to have it removed because he thought his health was normal, despite repeated reminders from the doctor and family. About two weeks before being admitted to the hospital, the patient had a fever, and a piece of the JJ catheter was found to have broken and dislodged from the colostomy bag.

At the Central Hospital for Tropical Diseases, CT scan results showed that stones were stuck around the JJ catheter, occupying almost the entire area of both renal pelvises. The right kidney had a large stone measuring 34 x 29 mm, the left kidney had a stone measuring 20 x 13 mm with many small scattered stones. The stones on the kidney caused grade 3 hydronephrosis. Worryingly, the patient's blood potassium index was up to 6.9 mmol/L (normal blood potassium is from 3.5 - 5.0 mmol/L), with the risk of cardiac arrest at any time. "Therefore, the patient was taken for emergency dialysis immediately," said Dr. Tran Duy Hien, Department of General Surgery, Urology and Andrology, Central Hospital for Tropical Diseases.
After the blood filtration process to return the potassium index to normal, the patient was given antibiotics to control the infection. Then, the patient underwent percutaneous nephrolithotomy on both kidneys. Percutaneous nephrolithotomy helps remove stones effectively, minimally invasive, thanks to which the patient recovered very well, did not need to do dialysis again, kidney function improved significantly, health stabilized after 5 days of treatment.
Doctor Tran Duy Hien recommends that patients after intervention to break up urinary, ureteral, and kidney stones are often placed with a temporary JJ catheter. It is very important to remove the JJ catheter on time. Patients must comply with the doctor's appointment to remove the catheter. In case of abnormalities after catheter placement or if the patient has not removed the catheter on time, they should contact the medical facility immediately to avoid leaving the catheter in for too long.
