On the afternoon of October 31, at the National Assembly House, continuing the 8th Session, under the chairmanship of National Assembly Chairman Tran Thanh Man, the National Assembly discussed in the hall the draft Law on amending and supplementing a number of articles of the Law on Health Insurance (HI).
Participating in giving comments, National Assembly Deputy Mai Van Hai, Provincial Party Committee member, Deputy Head of the National Assembly Delegation of Thanh Hoa province, basically agreed with the Government's Submission and the Social Committee's review report on the draft Law amending and supplementing a number of articles of the Law on Health Insurance.
Commenting on Article 12: Health Insurance Participants, the draft Law has added many health insurance participants in different groups, delegate Mai Van Hai said that it is very necessary to move towards the goal of universal health insurance coverage.
However, the proposal in the group funded by the state budget for relatives of subjects in the armed forces specified in points a and b, clause 3 needs to specify which relatives are eligible? It should be specifically regulated as biological parents of the wife (or husband); biological children, adopted children and legal guardians. The group supported by the state budget recommends that there should be more preferential policies for subjects such as students; subjects who are households engaged in agriculture, forestry, fishery and salt production with an average living standard; households who are ethnic minorities so that these subjects can all participate in health insurance. These are subjects that still face many difficulties.
In addition, it is recommended to consider adding state budget objects to pay health insurance for heads of mass organizations in villages, hamlets, and sub-regions. This is the opinion and wish of many voters through each voter contact session.
In Article 26: Registration and initial health examination and treatment under health insurance, delegate Mai Van Hai basically agreed with the amendments and supplements to the draft law on initial health examination and treatment registration, for initial and basic levels of health examination and treatment. However, the addition of regulations on initial health examination and treatment registration for specialized health examination and treatment facilities needs to be carefully considered and assessed for specific impacts.
Because, in reality, specialized medical facilities mostly have to treat patients with serious and terminal illnesses, hospitals are often overloaded, putting great pressure on medical staff and doctors; physical facilities do not meet the requirements, if regulations require additional tasks of initial medical examination and treatment for people under the health insurance regime, it will continue to create a burden for specialized medical facilities, which can be performed well at the initial and basic levels.
Delegate Mai Van Hai suggested that it should not be considered to regulate primary medical examination and treatment at the specialized level, but only to register for primary health insurance medical examination and treatment at the primary medical examination and treatment level, the basic medical examination and treatment level is appropriate. At the specialized level, it is only appropriate to consider regulating primary medical examination and treatment for a number of priority groups of subjects in the health care and protection of cadres and those who work directly at specialized medical examination and treatment facilities.
In Article 27: Transferring patients between health insurance examination and treatment facilities. In which, Clause 3 stipulates that people with health insurance cards go to the medical examination and treatment facility on their own, not at the place where they initially registered for health insurance examination and treatment, during the inpatient treatment period, if they discover another disease outside the scope of expertise of the medical examination and treatment facility or the condition progresses beyond the professional capacity of the medical examination and treatment facility.
Delegate Mai Van Hai suggested that it is necessary to consider allowing patients with health insurance to go to the hospital themselves, meaning they will not have to transfer to another facility, which is the wish of many voters. However, according to the delegate, it is also necessary to consider carefully, creating conditions for patients but also ensuring strict and convenient management, as well as medical examination and treatment of medical facilities.
Because, not transferring to another level so that patients can go by themselves will lead to some problems such as: creating impatience, not feeling secure about treatment at primary and basic medical facilities, leading to many patients being transferred to higher levels for treatment, leading to overload at higher levels, especially specialized levels, greatly affecting the medical examination and treatment of specialized medical facilities; moreover, patients with health insurance cards go by themselves, but not all patients know how to go to the right medical facility. Therefore, it is recommended that there should still be a procedure for transferring hospitals, but it must create the most favorable and fastest conditions for patients when they have to transfer.
In Article 31: Payment for health examination and treatment under health insurance. In which, Clause 4 stipulates payment for the cost of drugs and medical equipment transferred between health examination and treatment facilities under health insurance, such a provision is to facilitate patients, in case the treatment facility does not have drugs. However, this also needs to be considered further, and needs to be regulated more specifically, to facilitate the transfer and payment of drugs and medical equipment, to promptly serve the treatment of patients; if the transfer of drugs and medical equipment to serve patients is not timely, it will cause difficulties for patients.
To facilitate patients, delegate Mai Van Hai proposed to consider additional regulations on payment for patients if they have to buy drugs and medical equipment on the market according to a doctor's prescription, on the list of drugs and medical equipment, and the payment rate of the health insurance fund, which will make it more convenient for patients to receive treatment.
