Breast implant rupture after breast augmentation

Doctors at the Center for Plastic Surgery and Aesthetics, Viet Duc Friendship Hospital (Hanoi), have successfully treated a patient whose breast implant ruptured after breast augmentation with a large textured implant at a private cosmetic clinic, using the armpit endoscopic method.

Patient 43 years old, in Hanoi , said that she had breast augmentation surgery 5 years ago, at a private beauty salon. At that time, after surgery, the patient had a lot of pain, her breasts were bruised and tight, the doctor advised her to have a massage many months later so that her breasts would soften.

Recently, after the news that large textured breast implants were recalled in Europe and the US due to the possibility of capsular contracture or the growth of foreign cells around the implant, the patient returned to the cosmetic clinic where she had her first surgery and then went to Viet Duc Friendship Hospital for a check-up.

Associate Professor, Dr. Nguyen Hong Ha, Head of the Department of Plastic Surgery of the Hospital, said that the patient had an uneven surface of the breast implant, and was ordered to have an MRI scan using a 3.0 Tesla machine to diagnose breast diseases. The results showed that the breast implant had ruptured before but the patient did not know, so she was ordered to have surgery immediately.

After breast implants, if there are no abnormal signs, the patient only needs an ultrasound and annual check-up. If symptoms of pain or lumps appear, they should see plastic surgery specialists at major centers with accurate diagnostic equipment and specialized cell pathology, so that they can be treated, monitored, and cared for for complications of this type of implant in the long term.

The surgery was performed at the same incision (axillary) but using laparoscopic surgery.

Dr. Nguyen Hong Ha informed: "The surgery was very difficult because the rough, large bag shell had firmly adhered and stuck to the surrounding tissue." The surgeons spent nearly 2 hours dissecting to remove all the thick fibrous capsule and clean the liquid silicone that had spilled out. The rough fibrous capsule around the bag was also sent for pathological testing. Within 30 minutes, the patient's test sample was determined to have no malignant cells, and the doctors replaced the old, broken breast bag with the latest generation of breast bags, restoring the patient's soft, natural breasts like new.

According to Dr. Ha, recently, the US and European health authorities have simultaneously requested the recall of all these large textured bags on the market. "The above patient was quite lucky, he went to the hospital for examination and diagnosis, discovered the complication of the bag rupture and was promptly operated on to remove the ruptured silicone before it could spread far," said Dr. Ha, and shared: Surgeons who are not trained in endoscopic surgery will place breast implants using the old-style axillary incision. Currently, the endoscopic technique of placing breast implants through the armpit will not require a new incision on the chest, thereby significantly reducing complications of bleeding, hematoma, reducing pain as well as controlling the formation of fibrous contractures after surgery, reducing complications after breast implant placement.

“Large textured bags are always hard and have rough shells, so the breasts are difficult to soften after placement. Over time, the rough shell can stimulate the body to create a fibrous capsule, which can easily rupture and can also produce some strange cells around the capsule. For cases that have previously had surgery with large textured bags that are now being recalled from the market, they need to be re-examined and monitored regularly,” Dr. Ha noted.
